Free Introductory Mini-Session

November 21, 2009 by Dr Dane

This no-pressure option is available to prospective clients who are ready to make a commitment to work with a health care professional, and want to experience me and my work. It gives us both a chance to see if we might work well together.  This offer is only valid for new clients and limited to one mini-session per person.

The free visit is usually limited to 20 minutes. If your situation is fairly straight-forward (not complicated by other illness), we should have time for a brief consultation, evaluation and treatment – focused on your main area of concern. The evaluation will usually allow us to:

  • Discover a few of the hidden causes of symptoms you currently have
  • Identify potential problems before they become obvious
  • Feel the difference in this unique approach
  • See if my work will likely be helpful to you

This is not a “canned” exam, but is rationally and intuitively guided to explore  elements that are the most important for you. We will look for the hidden factors that are contributing to your situation.

This brief visit will allow you to taste the unique, multifaceted approach that helps so many people achieve their treatment goals.  It is not intended to cure everything you walk in with. Some additional assessment will probably be necessary to fill in the blank spots before beginning treatment, if you choose to take the next step.

Call 541-821-3263 to schedule your

complimentary 20 minute Mini-Session

Using Our Time to Greatest Benefit – Doing your part
In order to have time to do a brief exam, I ask you to fill out the short version of my New Client form and bring it with you (click link to open the form).   Please print & complete this form and bring it with you to your free 20-minute session.  This will save a lot of talking time if it is done well.

If your situation is more complex, I may have you fill out my regular New Client forms (two) instead of the short version. We can make that determination when we set your appointment time.

No Pressure
After your complimentary visit, if we both feel it would be appropriate to continue with a treatment session, it will usually be scheduled separately. There is no sales pitch or pressure – just straight, honest information. Your body will know. We create your plan together.

The free 20-minute session is usually done in person, but in some cases it may be done over the phone. People living out of the area who plan to travel to Ashland for care would be an example of an initial phone consult.

My Commitment to Clients

I’m committed to you as a client, and I want to offer you the necessary resources to attain your health goals. My mission is to serve you in a way that is not only technically correct and deeply effective, but that also heartfully supports you (if you’re interested) in becoming more self-empowered, healthier, happier, and expressive of your true potential.

I see my relationship with those who come to me as a sacred trust, between two human beings, to work toward a mutually defined and agreed upon goal.

I consider us to be partners in your health, each with our own responsibilities.

My job is to be a general support person, part detective/doctor, part manual therapist-healer, part coach, part cheerleader, part magician and part fool – laughter helps the healing process :-)

Your job is to care for yourself and follow my suggestions as well as you can, given your life circumstances, and to at least consider making some new choices if your circumstances are undermining your well-being.

My commitments to you, as part of your health care team, are:

  • To be worthy of your trust, to the best of my ability, as a doctor and fellow traveler on life’s journey.
  • To listen well to you and your body, giving you my complete attention during our time together.
  • To empower you in your own health, with valuable information and effective self-help practices.
  • To work with your existing health care team (if you wish), to the extent they are open and available, to further your best interest.
  • To tell you when I think we need another opinion, or expertise I don’t have, and to offer an appropriate referral.
  • To respect your right to confidentiality.
  • To respect your personal interests and beliefs.
  • To maintain good professional boundaries and to respect yours, without sacrificing human warmth and friendship.
  • To remember the Golden Rule.
  • If I should fail in any of the above, to readily acknowledge I’ve made a mistake.

Dr Dane

In the Beginning

Dr Dane at age 6: in love with nature, climbing trees, and reading animal stories

Dr Dane at age 6: in love with nature, climbing trees, and reading animal stories

I was halfway through the second grade when I decided what I wanted to be when I grew up.  I was going to be a scientist and a writer!  This desire apparently came from my fascination with nature and love of good books.  My favorite books usually had animals as the main characters.

After high school, I did a four-year tour in the US Navy, then returned to California for college.  At Sonoma State University, I studied biology, expressive arts and psychology.  Drawn to working with people, I then completed a training in massage  therapy and started a small practice in bodywork.

The Clouds Parted
I had forgotten about the scientist and writer, and was back to wondering what I was going to be when I grew up.  I felt there was something more yet to come.  Then one day a very intuitive friend said, “I keep getting this image of your hands – have you ever thought of being a chiropractor?”  To make a long story short: the clouds parted, trumpets sounded, and I knew I was going to chiropractic college.

Chiropractic College
Six months later, I was a student at Logan College of Chiropractic near St. Louis.  There, I learned about stress from first-hand experience as I went through the non-stop 4,000+ hour program.  I graduated in 1982, having paid for my Magna Cum Laude with a stress-related case of food allergies!

Expanding My Viewpoint
I moved to Minnesota, and passed their state board exams for my chiropractic license.  Shortly thereafter, I opened my own chiropractic office in a suburb near Minneapolis.  As a new practitioner, I soon received an education about taxes and overhead, while I continued my study of “stress.”

Because of my personal experience, I began to see my patients through new eyes.  I noticed how the stress in their lives seemed to be causing much of their spinal, muscular and other problems.

Curious about the mental-emotional aspects of healing, I explored NLP (Neuro-Linguistic Programming) and the deeply relaxing CranioSacral Therapy – even a bit of hypnosis.  I sought out healing approaches which incorporated the Body-Mind-Spirit model.  I talked with my patients about what was going on in their lives.  They liked it, and so did I.

Teaching
Eventually, I sold my practice and accepted a teaching position at Northwestern College of Chiropractic in Minneapolis.  I loved working with the students, and sharing what I’d learned.  During my ten years at Northwestern, I was certified in Acupuncture and earned an advanced certification in Diagnosis and Internal Disorders.

New Therapies Prove Successful
Over the years, I studied other disciplines offered by the Upledger Institute in addition to CranioSacral Therapy.  They included Lymph Drainage Therapy, Visceral Manipulation (gentle work with the organs and other soft tissues), Interactive Guided Imagery, and SomatoEmotional Release.

These modalities emphasize a sensitive touch and hold the client’s process in deep respect.  Each approach opens a new window through which to view the body-mind, and reveals another important factor in the healing process.  Best of all, they work extremely well when combined.

Personal and Spiritual Growth
I’ve been a long-time student of personal and spiritual growth, and was ordained as a minister with the interfaith Universal Brotherhood in 1998.  I’ve had a natural and growing interest in empowering others in the area of health and well-being for over 35 years.

Traveling Photographer
My wife and I left Minnesota in 2003 to travel, and landed in Ashland, OR in 2005.  Since 2003 I’ve been passionately into nature photography.  You can view the results in my online gallery, www.Inner-Light-Images.com.

Back in Practice
After this long vacation, I began to feel the call back to active practice again.  I dug out my books and sat for the Oregon chiropractic board exam.  I received my Oregon chiropractic license in Dec, 2008.

A Deeper Understanding of Stress

Learning from my own experience and that of my clients has led me to a deeper understanding of stress and stress-related health problems.  I learned about its causes & effects, and how it can be eased with natural therapies.  It can even used as a catalyst for personal and spiritual growth.  Stress plays a huge role in causing and perpetuating an estimated 90% of the health problems we face today.

I’ve published several articles on health and well-being, including The Art of Being Your Own Best Friend. This article has helped many people re-evaluate their priorities and reduce their stress levels.  You can browse these articles in the sections on Health Articles, Stress Mastery, All About Pain, and Therapies by clicking on these links, or using the Category List to the right.

Creations
I’ve taught advanced myofascial classes for massage & physical therapists.  I authored a personal & spiritual growth course with a focus on reducing stress.  It’s called Four Steps to Transformation, and includes a CD called Transformational Meditations.  I also led a six-session workshop in its practical application.

I’m currently working on a DVD entitled Enduring Peace, to facilitate heart-centered awareness, relaxation, stress-reduction and personal growth.  You can find out more about the DVD in Relax Now.

Dr Dane at 62, a health scientist who loves nature, and still climbs trees!

So, as the wheel of life has turned, that second grader may have known something his twenty-something self did not.  Here I am, being a scientist of sorts, studying healing and the human body, and writing to help people live happier, healthier lives.  That’s what Body-Mind-Peace is all about, and I feel blessed!

FAQ

May 18, 2009 by Dr Dane

What should I wear for my visits?
For Everyone:
Loose-fitting clothing in general is very helpful. Pants, sweats or shorts work well for men and women. Elastic waist bands are helpful, but not required. Tops should allow for easy access to the neck down to the collar bones (leave the turtle-necks at home).

For Women:
It’s best not to wear a dress, as I often use the legs for muscle testing or orthopedic tests.

For upper back work, reversing a blouse so it opens in the back usually works well. For shoulder work, a sports bra is an option. I have a couple of hospital-type gowns to use if necessary. They look dorky, but I won’t tell anyone!

Do I need to take off my clothes?
No. While I usually need to see the areas that are bothering you, wearing the type of clothing suggested above will allow for that 95% of the time. Whatever the case, your concerns will be respected.

Is your office handicapped accessible?
Yes. There is a small driveway and special parking place to the right of the front door, accessible directly from Siskiyou Blvd. The front entrance allows for wheel chair entry, and there are no steps inside. If you park in the lot in back of the building, use the sidewalk around the right side of the building. It’s wide enough for a wheel chair or walker.

How does your Sliding Fee Scale work?
My regular fee is $60 for a half-hour and $120 for a full hour.

If that is more than you can realistically afford on any particular visit, you are free to contribute what you can that day, down to $30 for a 30 minute visit, or down to $60 for a 60 minute visit.

For another option, please see “Fees & Insurance” under Dr Dane’s Practice for information on the package discount.

Do you take x-rays in your office?
No. I send people to Ashland Community Hospital for x-rays or lab work.

How many visits will it take for me to feel better?
This depends on many factors. Most of my clients experience significant improvement within 1-3 visits. After your free consultation, I can usually tell you if more visits will be needed to achieve your goals.

If pain is your main concern, you can browse through all about pain for an educational overview of it’s most common causes. You will find things here that you haven’t thought about!

Testimonials

A note about testimonials: because there so many factors which can cause a particular symptom, there is no guarantee that you would experience similar results for a similar problem. These reports are mainly here to give you an idea of the breadth and quality of my work. Only a personal, hands-on evaluation can provide you with a realistic idea of your own situation.

Bad Auto Accident
“Within a few days of a double roll-over in my car, I developed some serious neck, shoulder and low back/hip pain. I also began experiencing debilitating brain fog, short-term memory loss and difficulty concentrating, along with physical and emotional fatigue and a general malaise that were not at all normal for me.

“My massage therapist referred me to Dr. Dane Roubos, who treated with me with CranioSacral Therapy, energy healing, his special muscle work, and many other gentle therapies. He was patient, understanding and had a refreshing lack of agenda as he let my body lead the way.

“In just a little over 2 months I’ve regained at least 90% of my function, and feel like myself again. Working with Dr. Dane has been an uplifting and enlightening experience. I feel extremely grateful and would recommend his wonderful work to anyone!”

Paula Bandy, Ashland, OR

Constant Shoulder & Arm Pain
“After being in nearly constant pain for five years from a shoulder injury, I am almost free of it after just three visits, thanks to Dr. Dane. I worked with many other doctors over those years, including a chiropractic shoulder specialist, and none of them were able to bring an end to my pain. Dr. Dane did!

“My quality of life has been greatly improved as a result of his gifts and I strongly recommend him to anyone. You can’t miss when you find someone who is this skilled at getting to the root of the problem!”

Rev. Kelley Springer, Talent, OR

Low Back Pain
“I want to recommend Dr. Dane Roubos as an incredibly gentle, intelligent and effective healer for spine issues.

“I recently developed severe tension, spasms and radiating pain in and around my lower back. I have gone to chiropractors for years, and the usual experience would have been painful adjustments that probably would not hold for more than a couple of days.

“When I saw Dane, he had me lie on a comfortable massage table. He did some very gentle touch – working mostly through my lymph system and general energy to release the tight joints and muscles. It was so gentle that I actually fell asleep a couple of times. After I awoke and then rested for a while on the table, I got up and noticed an immediate difference. The pain had gone from an 8 to a 1 on a 0-10 scale, and the vertebrae that I had felt was out seemed to be straightened.

A few days later I did a 7 hour car trip – usually hard on my back – and noticed nothing. That was the surprising thing – I knew that I was better because I simply was not thinking about my back and had great energy at the end of the drive.

“One thing that struck me was Dane’s depth of knowledge and wisdom. I highly recommend his unique, caring, gentle and powerful work.”

Duane Light, Attorney, Ashland, OR

Shoulder & Arm Pain
“I had shoulder pain radiating into my arm for three weeks, and sought help from another natural health provider, with little change. I had to cut back on work due to declining range of motion and increasing pain.

“Then, in one session with Dane I was relieved of 95% of my symptoms. I learn more about myself each time I see him, due to his astute inquiries and thorough testing. After sessions, I feel deeply nourished and held in the highest regard. I appreciate being seen from a whole person perspective.

Dane holds a “safe space,” and I get to deeply experience my integrated self…body, heart, mind and spirit. I am so grateful for his help, and highly recommend his work!”

Holly MacGowan – Ashland, OR

Chronic Shoulder Pain & Limitation
“Chronic shoulder/rotator cuff pain has been limiting my arm movement for many years; an old athletic injury. Other practitioners have worked on it, giving improvement for a day or two, at best.

“After three visits with Dane (over two weeks), the pain is almost gone and the range of pain-free movement is greatly improved, much to my delight! The treatments were deeply relaxing, healing far more than just the shoulder, as he moved his gentle healing skill to the liver, lymphatics and other tissues related to the shoulder problem.

“He also gave dietary suggestions which I’ve found very helpful. This is good work!”

C. Martin, Entrepreneur – Ashland, OR

Back, Shoulder & Arm Injury
“I slipped on some ice and fell hard on my right elbow, injuring my arm, shoulder and back. I have 9 fused vertebrae, and my back doesn’t handle shock very well. Within three hours my back and arm hurt so much I felt nauseous with pain. I could hardly raise my arm, and had a 6 hour drive the next day.

“Dane went out of his way to make a house call, and gently untied the knots and greatly eased the pain in one session. I was able to make the drive with a functioning arm, using only a low dose of pain meds. I don’t think I could have done it without his help!”

Shaktari Belew, Holistic Educator, Ashland, OR

Comprehensive Approach, Nurturing Integration
“Dane is one of the best kept secrets in Ashland! After working in the field of Energy Healing and Human Mastery for over 30 yrs, I am grateful to find a ‘true healing professional’ with such a rich, comprehensive approach and mastery to work with.

“After each session I feel nurtured, fine tuned and so much more integrated with the spiritual acceleration I continuously go through. His work is a great blessing in a time when we are birthing a new Human.”

Diana Gazes, Mount Shasta, CA
www.thelighteam.com

Effective, Mulit-faceted Approach
“I have experienced many forms of body work and chiropractic care. As an acupressurist, I am very sensitive to energy and particular about how practitioners work with me. I have been extremely pleased with both the healing experience and the effectiveness of Dane’s work. He consistently got to the emotional and spiritual roots of the problems I came in with (various pains in my neck, joints and back).

“Dane’s approach addresses all aspects of healing and I rarely needed to come back for the same problem. I feel truly blessed to have found him and his work!”

Nancy Saslow, Acupressurist & Energy Healer, Minnetonka, MN

Chronic Low Back & Leg Pain
“By the time my acupuncturist referred me to Dr. Roubos, I had seen at least five specialists throughout the Midwest and various alternative therapy practitioners regarding my low back and radiating leg pain. I was soon able to walk without pain for the first time in eight months.

“Dane succeeding, where other therapists could not, is due to his deep, deep understanding of the human body, his ability to move seamlessly from one healing modality to another, and his spiritual understanding of the mind-body-soul connection. Dane is a true healer and I am forever grateful for his work with me.”

Kathleen Larsen, Retired Teacher, Northfield, MN

No One Like You
“I might be contemplating a visit to Ashland for treatment. I have found no one that can do the kind of work that you do, with the positive results. I find some therapists who almost have a piece of what you do, but the operative words are ‘almost’ and ‘a piece.’

“Isoloated, the pieces do not work as well as when they are applied in concert with each other . . . to say nothing of the healing “magic” you possess. The residents of Ashland are fortunate to have you practicing again!”

Kathleen Larsen, Retired Teacher, Minnesota

Stubborn Headache
“I suffered from a pretty severe headache for over a month and tried several things, including heavy doses of ibuprofen. Nothing worked. Within hours after seeing Dr. Roubos, my headache was gone and has not returned in months.

“Clearly, there was something physical going on, but it is very clear to me now that the energetic dimensions had as much or more to do with my condition as the purely physical aspects. Dr. Roubos treated both of these dimensions with great skill and care.”

Mark Langseth, College Administrator, St. Paul, MN

Chronic Hip and Leg Pain
“I had chronic hip and leg pain for which I had sought help from an orthopedic sports specialist, physical therapist (for six months) and a chiropractor (for 2 years).

“In just a few visits Dane took my healing further than the other three together. The pain is almost gone and my full flexibility is returning. I always leave his office feeling I’ve taken another step forward on my path to total health!”

Laura Erdman-Luntz, Co-Director, Center for Healthy Living, Eden Prairie, MN

Careful, Competent & Supportive
“Dane has been part of my health care team since 1988 and he’s the most careful and competent diagnostician and healthcare provider I’ve seen. He thoroughly explores the relationship of one symptom to another and to the whole person before considering treatment options.

“He skillfully utilizes approaches from multiple healing perspectives. I especially appreciate his support and creativity in designing self-help techniques for me to use so that I participate in restoring my own health.”

Bill Monson, College Administrator, Minnetonka, MN

Low Back & Leg Pain, with Swelling
“After a car accident, I developed severe pain in my low back, calves and ankles, along with swelling (edema) of both feet and ankles. I couldn’t stand very long, or walk very far. My medical doctors gave me pain pills and relaxants which relieved much of the pain, but not the edema or stiffness.

“Then I saw Dr. Roubos for 14 visits. After ten visits I was able to start dancing again. By the end, I was in better condition than before the accident, and the edema was gone. I want to stress the visits were 1 1/2 hours long, and all the work was done personally by Dr. Roubos. The results were amazing! I was 75 years old at the time.”

John Borden, Retired, St. Paul, MN

Lower Back Pain
“I had hurt my lower back in a fall. The pain was severe and I could barely put on my socks after a week. Dr. Roubos fit me into his schedule that very morning, despite being on his way out of town. I didn’t think one session would have such dramatic effects, but later that day I realized the pain had completely gone! As in other interactions with Dr. Roubos, he used a combination of physical and energetic work to treat my condition, and gave me greater insights into the possible causes beyond the obvious fall.

“He also discovered a chronic low back weakness, borne of a decades old injury, and provided me with helpful exercises to get that part of my back more fully integrated with the rest of my body. Clearly, Dane’s multi-faceted approach to treatment and prevention is a highly effective alternative care strategy.”

Mark Langseth, College Administrator, St. Paul, MN

Comprehensive Approach, Deep Relaxation
“Dr. Dane Roubos is an exceptional practitioner, blessed with the rare ability to combine the physical, mental-emotional and spiritual aspects of healing. He treats the physical body with an amazing range of techniques that target specific areas with precision.

“He also brings awareness to the subtle-energy body where imbalance often begins, restoring and supporting through imagery and self-discovery. During Dane’s sessions I always felt a great sense of calm which magnified itself by the end with a wonderful lightness that made me want to rest peacefully, the result of an astute practitioner.”

Dehanna Rice-Inkles, Yoga Instructor, NYC

General
In my experience with Dr. Dane Roubos, I have found him to be a
most generous, empathetic, and caring healer. He has been helping
me with some difficult issues with calmness, ease and
professionalism. He has the knowledge and experience in using many
different modalities, and his combination of intuition and practical
sense works to serve his clients very well.

Your First Visit

May 18, 2009 by Dr Dane

First, thank you for considering my job application!  Remember that I offer a free 20-minute Consultation.  This means that you can meet me in person and interview me for a position on your health care team.  Please see Free Consultation for more information.

First visits are usually one hour in length.  I prefer that you download the New Client Forms and bring the completed forms with you on that visit.   Suggested attire is in the FAQ section.

Also, please bring along copies of recent or significant lab tests, and any x-rays or other evaluations you may have of the area in question.

When you arrive, I’ll review your forms and ask a few more questions.  Then we’ll begin your evaluation.  The range of possible tests is enormous, so I can’t say much about it beyond this:  in addition to a good chiropractic assessment, I will also be checking several other important factors.  You can get a general idea of things we might be covering in the Overview – by scrolling down to the Evaluation and Treatment section.

I’ll check several things initially, and evaluate other while we go along.  Once I feel we have enough information to start working I’ll give you a synopsis of my findings and impression, and ask your permission to begin.

I’ll leave enough time at the end to re-test a few things to measure improvement, show you something you can do at home to help out, and answer any questions we have time for.

I’ll also ask permission to call you at home the next day to see how you’re doing, and see if you have any questions.

Unless you’ve already purchased a package, you’ll pay before you go, and if appropriate, I’ll give you a superbill to send to your insurance.

If you’re like most clients, you will probably be feeling better by now.  Depending on your situation, it could take longer.  My rule of thumb is that if you haven’t experienced a good improvement after three or four visits, we have probably missed something along the way.

We can delve deeper, or I can refer you to someone I think might help.  We can also keep working together while you’re getting input from other practitioners.  It’s your choice.  As long as I feel I can contribute something helpful, I’m open to continuing.

Home Visits

It’s difficult for some to get out of the house and make it into the office for a visit. Others simply prefer the privacy of their own home.

Another advantage of a Home Visit is to stay there and relax, instead of having to drive home after a deeply relaxing session.

One-hour Home Visits are available in the Ashland area for an additional fee of $15. If you have your own massage table set up, the fee is only $10.

Hours & Directions

Hours in the Ashland Office
I’m generally there from 2:00 pm to 6:00 pm, Mon, Wed & Friday. Appointments on other days and times are usually available when necessary. If you don’t have an appointment, it’s best to call first @ (541) 821-3263 to make sure I can see you.

Directions
We’re in Ashland’s historic Rail Road District, at 233 Fourth St, between B and A Streets, on the left as you’re heading away from downtown. There is on-street parking on both sides.

233 Fourth St - the old Arura Clinic Building

Our building is between the Illahe Gallery and the Peerless Inn.  An oval sign on the front of the building (near the top) says “Arura Clinic,” and the street address is on the lower part of the front doors. The adobe-style building is a light peach color.

We share the building with other independent healing arts practitioners, and it has a warm, quiet atmosphere. Bottled spring water is available in the waiting area. If our room (#3) is open, come on in. If not, please have a seat and I will check for you when I’m ready.

If You Get Lost
Please call me @ (541) 821-3263.  This phone if usually off when I’m in session, but please leave a message and I will get back to you as soon as I can.

Handicap Access
The building is handicap accessible, and everything is on one level inside.  When necessary, I can usually work with someone in a wheel chair and still achieve good results.

House Calls
These are available by special arrangement. We will need enough open space to set up a massage table and move around it on all sides. A quiet, private environment is best for treatment sessions.

(Ashland House Calls are an additional $10 if you already have a massage table set up, and $15 if I need to bring mine).

I also accept credit cards via PayPal.  Sorry, I do not carry a balance for clients who are paying cash. Please see Insurance, below if you have insurance.

Package Option – Save another 10%
Clients have the option of purchasing treatment time in advance at a discount. You can purchase $500 in treatment time for $450.

This package is offered to support those who are interested in improving their health on a deeper level than 2 or 3 visits would allow.

It’s a great deal for clients who need more than a few visits, or those who wish to come in on a regular basis for on-going support.  I recommend that you have 1 or 2 visits first, to make sure it’s a good fit for you.

Cancellation: if a client wishes to receive a refund for unused package time, they will relinquish the 10% discount on the time they’ve already used, and pay a $10 processing fee.  They’ll receive a check for the balance.

Free Evaluation & Treatment Mini-Session
This is available to prospective clients who want to meet me and get a sense of how I might approach their personal health situation. It gives us both a chance to see if we might work well together. Please fill out the short version of the New Client form and bring it with you.  If you cannot print internet forms, we will arrange for you to fill out the form in the office.

The free 20-minute session is usually done in person.  This allows us to actually test a few things and usually give you a “mini-treatment” so you can experience a little of my work directly before you make a decision.

In some cases, a verbal consultation may be done over the phone. People living out of the area who plan to travel to Ashland for care would be an example of an initial phone consult.

Personal Injury Ins (Auto Accidents, Injuries in stores, etc)
The only insurance I currently accept “assignment” on.  It includes any auto-related injuries, even if you wack yourself with the car door getting out. Stores and property owners also have personal injury policies which cover injuries on their property.

Assignment means that I do all the billing in these cases, and wait for payment. You must report the accident to the insurance company, and get a claim number before we do any evaluation or treatment.

Regular Health Insurance
You pay for the visit at the time (20% discount), and I provide you with a statement that has the necessary codes. You can submit this to your insurance company for reimbursement. There is no extra charge for this, unless the insurance company requests a report, etc.

If you plan to use this option, you can print my Insurance Coverage Worksheet and use it to call your insurance company with specific questions. This will tell us which procedures are actually covered by your specific policy, and other “fine print” information. It’s worth the time to do this, because it will help us avoid unpleasant surprises.

If your insurance company requests any report or paperwork related to a statement, I will need to charge you for the time to complete it – but only with your authorization.  I would give you an estimate based on what they’re asking for.

Medicare only pays for chiropractic spinal adjustments. They do not cover any of the wonderful therapies I offer, which actually reduce the number or frequency of adjustments you need to reach your goal. So I would rather give you a good discount along with highly effective therapies, skip the hassle of billing with Medicare, and pass the savings on to you.

Cancellation and Missed Appointment Fees
There is a $25 fee for canceling a visit the same day.

OOPS! You forgot your appointment altogether, and didn’t call Dr. Dane!  You are responsible for your scheduled visit time at a rate of $60 per hour. This is pretty lenient compared to most offices.

Showing up late for your visit
The clock starts running at your appointment time, so you will just have a shorter visit that day, but pay the usual fee for your scheduled visit.  Here’s the fun part – if I’m late for our visit, I will make it up to you in time on that visit or another day.

Most of us don’t care to fill out “yet another form.”  I know – I don’t like them either.  But there are some great benefits to completing my forms:

  • It saves us time, and saves you money.  The information given allows me to see a “bigger picture” of your health, and still focus on your chief concerns.
  • It aids our detective work and  allows me to address your main concerns more effectively.  We often find clues as to why those symptoms are there in the first place, so we can address the underlying causes.
  • This information often helps us spot potential problems before they become apparent.  An “ounce of prevention” is “worth it’s weight in gold” (which is about $1200 an ounce these days)!

Initial forms for new clients can be downloaded and printed with the links below. Once you’re looking at the form (a PDF file), click on the little “floppy disk” icon just above the upper left corner of the page. Then, from the window that appears, choose the location on your computer that you would like to save it to.

If you aren’t able to download or print with the computer you’re using, you can come  early to your appointment and fill them out then

For Most New Clients – please print & fill out both of these forms:
